Complete this survey to help us identify health priorities for residents to develop a community health improvement plan.
As a nationally accredited local health department, the Cabell-Huntington Health Department places high priority on collecting information from Cabell County residents to understand health issues, access to health care services, and programs or services that affect health.
We want to understand what is important to the community. The responses you provide are anonymous and will be shared with the community when the survey is completed. Results will also be used to identify health priorities for residents to develop a community health improvement plan.
