Chronic Disease Self Management

The Chronic Disease Self-Management Program is a series of workshops offered once a week, for six weeks, for two hours by two trained leaders. These workshops are for people with a chronic condition or those who care for someone with a chronic condition.

Topics that are covered in these workshops include:

  • Ways to deal with frustration, fatigue, pain and isolation

  • Exercises for maintaining and improving strength, flexibility, and endurance

  • Medications

  • Communicating with family, friends, and health care professionals

  • Nutrition

  • Decision making
